CARE COORDINATOR

Comprehensive Healthcare is seeking a Care Coordinator to provide services and coordination to clients with chronic health conditions in the Tri-Cities area. The Care Coordinator will work with clients to develop individualized health action plans and provide education and coaching on medications and health behavior changes. The role requires strong interpersonal and organizational skills, as well as comfort with addressing medical topics and conducting home visits.

Requirements

• Bachelor's degree in social work, psychology, or related field
• Eligibility for registration as Agency Affiliated Counselor
• Valid driver's license and vehicle liability insurance
