Care Coordinator - Health Homes

Comprehensive Healthcare is seeking a Care Coordinator to provide services and coordination to clients with chronic health conditions in Kittitas County. The Care Coordinator will work with clients to develop an individualized health action plan and coordinate with primary care providers and other treatment teams to facilitate communication.

Requirements

• Bachelor's degree in social work, psychology, or related field
• Must be eligible for registration as Agency Affiliated Counselor
• Must have valid driver's license and vehicle liability insurance
