Comprehensive Healthcare seeks a Care Coordinator - Health Homes to provide care coordination services for individuals with chronic health conditions in Ellensburg, WA. Requirements

• Bachelor's degree in social work, psychology, or related field
• Eligible for registration as Agency Affiliated Counselor
• Valid driver's license and vehicle liability insurance
