CASE MANAGER - Community Support Rehab

Comprehensive Healthcare is seeking a Case Manager to provide support and coordination of care to our outpatient clients in Yakima, WA. The ideal candidate will have excellent communication and relationship-building skills, knowledge of community resources, and comfort with making home visits and traveling locally.

Requirements

• Bachelor's degree in behavioral science or related field
• Must be eligible for registration as Agency Affiliated Counselor
• Valid driver's license and vehicle liability insurance
