Case Manager

Comprehensive Healthcare is seeking a Case Manager to provide support and coordination of care to outpatient clients. The Case Manager will work with clients to build skills necessary for independent living and connect them with community resources. The ideal candidate will have excellent communication and relationship-building skills, knowledge of community resources, and comfort with making home visits and traveling locally.

Requirements

• Bachelor's degree in social work, psychology, or related field
• Must be eligible for registration as Agency Affiliated Counselor
• Must have valid driver's license and vehicle liability insurance
