Job Description
The Case Manager is responsible for determining eligibility for client participation in coordinated services, conducting intake and assessment processes, developing individualized service plans, and documenting case notes and program activities.
Requirements
- Undergraduate degree in social work, psychology, or behavioral science
- Graduate degree in related field preferred
- Minimum of 1-year successful experience in case management
- Excellent organizational, verbal, and written communication skills
- Ability to adapt to a dynamic and changing environment
- Ability to travel to various sites daily
