Case Manager

Metropolitan Family Services is hiring a full-time Case Manager to join their Calumet Team for the Upward Bound program. The Case Manager will conduct comprehensive client assessments, develop service plans, and provide advocacy and supportive counseling to individuals, families, and groups.

Requirements

• Bachelor's Degree in Social Work or related field
• One year of case management experience in a social service or healthcare setting
• Valid driver's license and proof of insurance
• Ability to work effectively with diverse individuals and groups
