Care Coordinator - Social Worker

Concerto provides comprehensive care to the elderly and patients with complex needs. The Care Coordinator will work collaboratively with a team to provide medical, behavioral, psycho/social care and coordinate long-term services and supports.

Requirements

• Licensed Physician Assistant, Licensed Nurse Practitioner, Licensed Registered Nurse (RN), Licensed Bachelor Social Worker (LBSW), Licensed Master Social Work (LMSW)
• 1-3 years of experience in a healthcare, patient-oriented customer service position
• Experience with the populations similar to the Medicare Enrollee population, including community-based and facility-based people with medical, LTSS and/or behavioral needs
