Care Coordinator

Care Coordinators coordinate the day-to-day implementation of contractual requirements within the state’s Medicaid guidelines. Duties include training, enrolling, reporting, auditing, and complying with contract deliverables as well as ensuring company best practices are followed and upheld. The Support Broker/Consultant works with members on plan/budget development, submission, and revision.

Requirements

• Bachelor’s degree in the field of Social Work, Psychology, Counseling, Rehabilitation, Nursing, Sociology or Related Field preferred
• 6 years’ experience serving individuals with disabilities preferred
• Must have reliable transportation. If the employee will be driving, a valid driver’s license and auto insurance is required
• Combination of education and experience
• Ability to work flexible and/or extended hours, if needed, to meet the job requirements
