Job Description
The ECM Care Manager (LVN) facilitates patient continuity of care with the ECM healthcare team. This role involves coordinating patient needs, managing care plans, and encouraging patient engagement in health improvement activities. The LVN will engage patients and support their individualized care plans while ensuring that they adhere to clinical guidelines and contribute to high quality patient care.
Requirements
- Current Vocational Nurse License
- Two (2) + years of experience in one or more of the following settings:
- Valid Driver’s License
- Ability to travel
