Job Description
This role focuses on providing oversight to a Community Health Worker (CHW) team, evaluating and approving their assessments. The NP/PA will work collaboratively with physicians and navigate patient care gaps to improve member quality of life and reduce healthcare costs. The position involves clinical duties, health promotion, and leading a team of health professionals.
Requirements
- Licensed in California as NP or PA
- National Board Certification
- Graduate from accredited Masters or Doctoral Program

- Strong experience in patient care assessment
- Excellent verbal and written communication skills
- Ability to exercise sound judgment and discretion
- Bilingual abilities (Spanish, Chinese, Creole)
