Behavioral Health Prescriber (MSN/PA)

Crossroads Health is seeking a part-time Behavioral Health Prescriber to provide psychiatric assessments and medication management in their outpatient Youth and/or Adult Psychiatry setting. The Prescriber will engage with a collaborative team, using a Trauma Informed approach to offer integrated services for recovery and mental health.

Requirements

• Master's Degree (minimum)
• Licensed in the State of Ohio as a Physician Assistant, Psychiatric Nurse Practitioner, or Physician
• Controlled Substance Registration Certificate
• Medicare Provider Number
• Professional Liability Insurance
• Standard Care Arrangement (annually) or Supervision Agreement (whichever applicable)
• BLS/First Aid (employer provided)
• Valid driver's license
