Behavioral Health Prescriber (MSN/PA) - Youth

Crossroads Health seeks a Behavioral Health Prescriber (MSN/PA) - Youth to provide psychiatric assessment and medication management in the outpatient Youth Psychiatry setting.

Requirements

• Master's Degree (minimum)
• Licensed in the State of Ohio as a Physician Assistant, Psychiatric Nurse Practitioner, or Physician
• Controlled Substance Registration Certificate (DEA registration fees and licensure paid by agency)
• Medicare Provider Number
• Professional Liability Insurance.
• Standard Care Arrangement (annually) or Supervision Agreement (whichever applicable)
• BLS/First Aid
• Valid driver's license
