Job Description
As a Registered Nurse Case Manager, you will provide direct care to patients in their homes or care facilities, building trusting relationships and serving as the primary communication link within our interdisciplinary team. We are committed to ensuring that end-of-life care feels personal and compassionate, and every individual deserves to be treated with dignity, compassion, and respect.
Requirements
- 1 year of prior nursing experience
- An active unencumbered state Registered Nurse license
- Current driver's license and auto liability coverage
- Interest and ability in providing patient care in homes and facilities
