Registered Nurse (RN)

We are seeking a Registered Nurse Case Manager to provide direct care to patients in their homes or care facilities, building trusting relationships and serving as the primary communication link within our interdisciplinary team. Requirements

• Six (6) months minimum prior skilled nursing experience
• Active, unencumbered state Registered Nurse license
• Current driver's license and auto liability coverage
