Job Description
Provide direct care to patients in their homes or care facilities, building trusting relationships and serving as the primary communication link within an interdisciplinary team of holistic care providers. Join a team committed to ensuring end-of-life care feels personal and compassionate.
Requirements
- Six (6) months minimum prior skilled nursing experience
- An active, unencumbered state Registered Nurse license
- Current driver's license and auto liability coverage
- Interest and Ability in providing patient care in homes and facilities
