We are seeking a Care Coordinator to assist in the facilitation of recovery services for patients with opioid use disorder. The Care Coordinator will meet with patients, utilize collateral contacts, and complete documentation to ensure patient engagement with services. Excellent training and mileage reimbursement are provided.
Requirements
- Bachelor's degree in Social Work, Psychology, Psychiatric Rehabilitation, Sociology, Counseling, Vocational Rehabilitation, or Human Services Counseling
- At least one year of experience providing services to individuals with a diagnosis of substance use disorders or a minimum of one year of clinical experience working with individuals with cooccurring diagnoses of substance use disorders and mental illness
- Licensure as a Registered Nurse with at least one year of direct experience providing services to individuals diagnosed with a substance use disorder or a minimum of one year of clinical experience working with individuals with cooccurring diagnoses of substance use disorders and mental illness
- Board of Counseling Certified Substance Abuse Counselor (CSAC) or CSAC-Assistant under supervision of a CSAC or another supervisor as defined in 18 VAC 115-50-10
