Job Description
Crossroads Treatment Centers is seeking a Care Coordinator to assist in the facilitation of recovery services and ensure that all recovery activities are properly documented in the patient’s record. The Care Coordinator will work in collaboration with a buprenorphine-waivered practitioner and perform other duties as assigned by the Operations Director of OBOT Services.
Requirements
- Bachelor’s degree in Social Work, Psychology, Psychiatric Rehabilitation, Sociology, Counseling, Vocational Rehabilitation, or Human Services Counseling
- 1 year of experience providing services to individuals with a diagnosis of substance use disorders or a minimum of 1 year of clinical experience working with individuals with cooccurring diagnoses of substance use disorders and mental illness
- Licensure as a Registered Nurse with 1 year of direct experience providing services to individuals diagnosed with a substance use disorder or a minimum of 1 year of clinical experience working with individuals with cooccurring diagnoses of substance use disorders and mental illness
- Board of Counseling Certified Substance Abuse Counselor (CSAC) or CSAC-Assistant under supervision of a CSAC or another supervisor as defined in 18 VAC 115-50-10
