Job Description
The Mobility Program Counselor will provide information and resources to families to prepare them for a move in a range of neighborhoods. The Counselor will provide motivational counseling and ensure that all participant information is entered in SharePoint. Additionally, this position will include general admin support tasks.
Requirements
- Assists with recruiting families and owners to participate in the Mobility Counseling program.
- Enter intake survey data into SharePoint and schedule intake assessments.
- Provides pre-counseling, apartment search assistance to assist families to move into opportunity areas.
- Assists tenants in developing Personalized Action Plan (PAP) and make referrals based on identified needs.
- Monitors tenantâs PAP for goal completion and update Share Point.
- Effectively communicates with Mobility Counseling Program Director and Coordinator regarding any participant related concerns.
- Regularly updates the record-keeping system regarding family attributes.
- Participate in regular meetings to provide updates on progress, challenges, and successes.
- Safeguards confidential and sensitive information as required by program rules.
