Job Description
The Care Manager will provide case management assessment, person centered planning, and documentation, referral and linkage, and monitoring/follow-up to ensure the recipient's health and safety risks are assessed and their needs are met.
Requirements
- Bachelor's degree in a human service field
- 2 years of MH/SA/DD experience
- Licensed RN with 2 years of MH/SA/DD experience
