Care Manager

Care Manager provides case management assessment, person centered planning, and documentation, referral and linkage, and monitoring/follow-up. Ensures metrics for outcomes are met.

Requirements

• A bachelor's degree in a human service field
• Two years of experience in MH/SA/DD with the population served
• Licensure as a RN with two years of MH/SA/DD experience
