Care Manager Extender

The Care Manager Extender provides care management functions, documentation, referral and linkage, and monitoring/follow-up under direct and indirect supervision.

Requirements

• High school diploma or equivalent
• Certified Peer Support Specialists or Community Health Workers or Community Navigators or Parent/Guardian of an individual with I/DD or TBI or behavioral health condition or Lived experience with I/DD or TBI or behavioral health condition
• 2 years of paid care management type experience with at least 1-year paid experience at any time with population served
