Care Manager Extender

Care Manager Extender will provide care management functions, documentation, referral and linkage, and monitoring/follow-up to support individuals with behavioral health needs.

Requirements

• High school diploma or equivalent
• Certified Peer Support Specialists
• Community health workers (CHW)
• Community Navigators
• Parents or guardians of an individual with an I/DD or a TBI or a behavioral health condition
• A person with lived experience with an I/DD or a TBI or a behavioral health condition
• 2 years of paid care management type experience with at least 1 year paid experience with population served
