Care Manager Extender

Under direct and indirect supervision, provides care management functions, documentation, referral and linkage, and monitoring/follow-up for individuals with behavioral health needs.

Requirements

• High school diploma or equivalent
• Certified Peer Support Specialists
• Community health workers (CHW) or Community Navigators experience
• Lived experience with an I/DD or a TBI or a behavioral health condition
• 2 years of paid care management type experience
