Job Description
Care Manager provides case management assessment, person centered planning and documentation, referral and linkage, and monitoring/follow-up for clients. Ensures metrics for outcomes are met.
Requirements
- Bachelor's degree in a human service field
- 2 years MH/SA/DD experience with the population served
- Ability to complete TCM trainings
