The Complex Case Manager provides nuanced case management engagement for solutions to housing placement for people experiencing homelessness with complex high acuity needs or chronic homeless designation. The role involves developing case plans, identifying action steps, and monitoring progress through scheduled follow-up meetings, as well as collaborating with other community providers and advocating for clients towards a successful transition into long-term programs or sustainable housing.
Requirements
- Bachelor’s degree in social work-related field OR two years of related work experience
- Colorado Certified Addiction Specialist (CAS) certification or equivalent
- Active Driver’s License and insurable under Denver Rescue Mission’s standards
- CPR certification within 90 days of hire
- Must be a Christian holding to the beliefs set forth in the Denver Rescue Mission Statement of Faith
- Excellent verbal and written communication skills
- Ability to resolve conflict professionally and respectfully
