Job Description
Elite DNA Therapy Services is seeking a part-time Peer Navigator to support patients in behavioral and mental health services in Florida. This role focuses on improving patient engagement in outpatient healthcare, assisting in treatment adherence, and educating patients about their options. The position involves working within an integrated treatment team and maintaining confidentiality of patient information.
Requirements
- Build relationships with patients/members and the treatment team
- Educate patients about role as Peer Navigator
- Facilitate appointment scheduling
- Work with patients one-on-one to address discharge planning
