Job Description
Join Our Team as a Full Time Case Manager! EAC Network, a leading not-for-profit social service agency, is looking for a dedicated full time Case Manager within our Expanded In-Home Services For the Elderly Program.
Requirements
- Bachelors degree in a human service field
- Ability to communicate with and understand the needs of the elderly
- Must be able to lift 30-50 lbs. if necessary
- Must possess knowledge of Nassau County services and needs of the geriatric population
- Must be proficient in computer applications
- Must have access to automobile and possess valid NYS Driver's License
