Case Manager - In Home Respite

Join Our Team as a Case Manager! EAC Network is looking for a full-time Case Manager to join the Respite Companion program. As a Case Manager, you will provide case management support for caregivers and assist with day-to-day operations of the In-home Respite Services program.

Requirements

• Associates Degree required, Bachelor’s Degree in human services or related field preferred
• Knowledge of resources and entitlements for senior citizens and prior experience working with the elderly preferred
• Must have excellent communication skills, both oral and written
• Must be proficient in computer applications or other automated systems such as Microsoft Office Applications, email, and data base software necessary to preform work assignments
• Must have valid New York State Driver’s License and have access to an automobile
