Nurse Practitioner PCP PACE

Full-time nurse practitioner or physician assistant position available to provide primary care services to frail participants of the Neighborhood PACE program.

Requirements

• Full-time position, 40 hours per week, Monday - Friday, 8 hours per day
• Geriatric primary care services for PACE participants across multiple long term care settings
• Spanish proficiency preferred but not required
• Adult or Family Medicine certification considered
