Health Home Plus Care Manager

The Health Home Plus Care Manager partners with individuals living with mental illness, substance use disorders, and physical health challenges to provide comprehensive care management.

Requirements

• Provide the six core Health Home services
• Engage and retain individuals with complex needs
• Utilize evidence-based practices
• Maintain a caseload of up to 20 individuals
• Conduct comprehensive assessments and screenings
• Arrange for additional assessments as needed
• Navigate the healthcare system effectively
• Support individuals through coordination of care and advocacy
• Develop and monitor person-centered Plans of Care
• Facilitate effective transitions of care
• Engage family members and service providers in the care coordination process
• Maintain accurate and timely documentation
• Participate in scheduled supervision and required trainings
• Participate in on-call rotation and coordinate services among internal and external providers
• Communicate effectively with supervisors and team members
