East House is seeking Service Coordinators for a dedicated non-profit organization committed to supporting individuals on their journey to recovery. The role involves partnering with clients to identify and maintain access to treatment, housing, and community services. The goal is to foster a supportive environment and help individuals achieve stable housing.

Requirements

• . Partner with clients to develop individualized service plans based on client-identified goals.
• . Coordinate and advocate for access to mental health, substance use, physical health, financial, and other community resources.
• . Provide support in the community and in client homes, utilizing clinical knowledge and sound judgment.
• . Participate in case reviews and planning to ensure quality and stability of client services.
• . Support client access to housing, including landlord relations, lease retention, and financial assistance.
• . Assist clients with benefit applications, budgeting, and payee agency coordination.
• . Promote family well-being through school, recreation, and healthcare connections.
• . Provide medication support as required by the program.
• . Maintain documentation in accordance with funder and agency standards.
