Job Description
Easterseals Blake Foundation is seeking a Supervisor- Direct Support Professional/ Professional Caregiver to provide support and assistance to individuals with intellectual, physical and/or developmental disabilities to live as independently as possible.
Requirements
- High School Diploma/GED with 2 years of supervision required
- Associate’s degree preferred
- Bilingual (Spanish) preferred
- Regulatory requirements
- Ability to obtain and maintain an AZ DPS Level 1 fingerprint clearance card
- Ability to obtain and maintain CPR, First Aid, Article IX, and Prevention & Supports certification
- Current, unrestricted drivers license with no more than two (2) minor moving violations or one (1) accident within the past three (3) years
- Ability to pass a criminal background check
- Must be at least 21 years of age
- Three (3) years of driving experience required
