Job Description
At Elara Caring, we have a unique opportunity to play a huge role in the growth of an entire home care industry. We are looking for a Registered Nurse Case Manager to join our team, providing high-quality care to patients in their preferred home setting.
Requirements
- Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing
- Current State License as a Registered Nurse RN
- 1 year of experience in a clinical care setting
- Experience in a hospice or home health environment is preferred
- Ability to frequently lift, push, pull, and support up to 50 pounds
- Valid driver’s license and insurance and reliable transportation
