Job Description
We are seeking a Registered Nurse Case Manager to join our team at Elara Caring. As a Registered Nurse Case Manager, you will contribute to our success by assessing patient needs, communicating with healthcare teams, and providing support to patients and families. Requirements
- Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing
- Current State License as a Registered Nurse RN
- 1 year of experience in a clinical care setting
- Experience in a hospice or home health environment is preferred
- 50% travel required
- Ability to frequently lift, push, pull, and support up to 50 pounds
