Job Description
Elara Caring seeks a Registered Nurse Case Manager to join its team, providing high-quality care to patients in their preferred home setting. The role involves assessing patient needs, communicating with the clinical team, and providing support at the time of death. Requirements
- Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing
- Current State License as a Registered Nurse RN
- 1 year of experience in a clinical care setting
- Experience in a hospice or home health environment is preferred
