Job Description
We are seeking a Registered Nurse Case Manager to join our team at Elara Caring. As a Registered Nurse Case Manager, you will contribute to our success by assessing hospice care patients and families, communicating with the clinical supervisor and physician, and providing support at the time of death. You will also teach hospice philosophy and pain and symptom management to staff in alternate care settings.
Requirements
- Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing
- Current State License as a Registered Nurse RN
- 1 year of experience in a clinical care setting
- Experience in a hospice or home health environment is preferred
- 50% travel required
- Ability to frequently lift, push, pull, and support up to 50 pounds
- Valid driver's license and insurance and reliable transportation
