Hospice Registered Nurse - Case Manager

We are seeking a Registered Nurse Case Manager to join our team at Elara Caring, a pioneer in delivering high-quality home care. As a RN Case Manager, you will contribute to our mission by providing compassionate care to patients and families, assessing patient needs, and communicating with the clinical team.

Requirements

• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing
• Current State License as a Registered Nurse RN
• 1 year of experience in a clinical care setting
• Experience in a hospice or home health environment is preferred
