Job Description
Elara Caring is seeking a Hospice Registered Nurse Case Manager to join their team. The role involves assessing patients' physical, psychosocial, and environmental needs, communicating with clinical supervisors and physicians, and providing support to patients and their families. The ideal candidate will have a graduate degree in nursing, a valid state license, and experience in a clinical care setting.
Requirements
- Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing
- Current State License as a Registered Nurse RN
- 1 year of experience in a clinical care setting
- Experience in a hospice or home health environment is preferred
- Ability to frequently lift, push, pull, and support up to 50 pounds
