Patient Care Coordinator

The Patient Care Coordinator serves as a key point of contact for patients and their families, supporting the coordination of care by scheduling appointments, managing patient information, and facilitating communication between patients, providers, and care teams.

Requirements

• High school diploma or GED required
• 12 months of customer service experience or 6 months of healthcare experience in a similar role required
• Bachelor's degree, or enrollment in a bachelor's degree program, may be considered in lieu of required experience
