Job Description
Ennoble Care is seeking an experienced Advanced Provider (Nurse Practitioner or Physician Assistant) to deliver primary and palliative care to residents at an Assisted Living Facility in Long Island, NY. This position offers a convenient, in-person experience, eliminating travel challenges. The provider will be a key clinical member, applying medical principles and collaborating with a multidisciplinary team to manage the needs of geriatric patients.
Requirements
- Board Certified, NP or PA in NY
- Strong clinical skills
- Positive demeanor and strong people skills
- Ability to independently solve problems
- 1-2 years primary care experience (hospice preferred)
- Understanding of geriatric syndromes and chronic disease management
