Care Coordinator, Embedded (Reston, VA - Warrenton)

Ennoble Care is seeking a full-time Care Coordinator, Embedded to join our team, providing on-site support within one or more partnered long-term care facilities in the Reston, Virginia area, with a primary focus on the Warrenton vicinity.

Requirements

• Legally authorized to work in the United States of America
• Active, unrestricted clinical certification or licensure
• Minimum 1 year of patient-facing experience in a long-term care, skilled nursing, or similar clinical setting
• Comfortable communicating proactively and professionally
• Ability to pass a background check and drug screen
