Care Coordinator, Embedded (Reston, VA)

Ennoble Care is seeking a full-time Care Coordinator, Embedded to join our team, providing on-site support within one or more partnered long-term care facilities in the Reston, Virginia area.

Requirements

• Legally authorized to work in the United States of America
• Active, unrestricted clinical certification or licensure
• Minimum 1 year of patient-facing experience in a long-term care, skilled nursing, or similar clinical setting
• Ability to work full-time, Monday through Friday, 8:00 a.m. to 4:30 p.m. EST, in an on-site role, based in the Reston, Virginia area
