Job Description
Ennoble Care is seeking a full-time Care Coordinator, Embedded to join their team, providing on-site support within one or more partnered long-term care facilities in the Reston, Virginia area, with a primary focus on the Front Royal vicinity. This role is an integral, in-person member of their clinical care team, working alongside Ennoble Care providers in a family-practice-style model of care that emphasizes continuity, relationship-building, and patient-centered support.
Requirements
- Legally authorized to work in the United States of America
- Active, unrestricted clinical certification or licensure
- Minimum 1 year of patient-facing experience in a long-term care, skilled nursing, or similar clinical setting
- Ability to work full-time, Monday through Friday, 8:00 a.m. to 4:30 p.m. EST
