Care Coordinator, Embedded (Reston, VA - Winchester)

Care Coordinator, Embedded job description. The role is to provide on-site support within partnered long-term care facilities in the Reston, Virginia area, with a primary focus on the Winchester vicinity. The ideal candidate is a clinically certified individual with at least one year of patient-facing experience in long-term care, skilled nursing, or similar settings.

Requirements

• Legally authorized to work in the United States of America
• Active, unrestricted clinical certification or licensure
• Minimum 1 year of patient-facing experience in long-term care, skilled nursing, or similar clinical setting
• Comfortable communicating proactively and professionally
• Cultivate and maintain positive relationships with residents, families, providers, and facility staff
• Well-developed organizational and time-management skills
• Strong working knowledge of healthcare terminology and care coordination principles
• Proficiency with Microsoft 365
• Ability to work full-time, Monday through Friday, 8:00 a.m. to 4:30 p.m. EST
• No weekends, no on-call, daytime hours
