Job Description
Ennoble Care is seeking a Memory Care Coordinator to provide care coordination and support services for patients and their care partners. The ideal candidate will have experience in dementia care and strong communication skills.
Requirements
- Experience working with individuals diagnosed with dementia or other memory-related conditions
- Familiarity with basic healthcare terminology, conditions, and care principles
- Knowledge of local resources for dementia patients and care partners
- Comfortable with frequent phone, video, and digital communication
- Strong verbal and written communication skills
- Proficiency in electronic medical records (EMR), email, and virtual communication platforms
