Job Description
We are seeking a qualified and licensed Entitlements Case Manager to provide case management services on an independent contractor basis. The contractor will schedule and facilitate group and individual case management sessions, conduct case management assessments, and complete client documentation within the designated service period.
Requirements
- Bachelor's in psych/social work/counseling/nursing or related (Master's preferred)
- Licensure/Certification (one or more): LCADC, LGADC, CAC-AD, CSCAD, ADT, LCPC, LGPC, LCSW-C, LCSW, LMSW, LBSW, LCMFT, LGMFT, Licensed Psychologist, Registered Psychology Associate, CRNP-PMH, APRN-PMH, CNS-PMH, or eligible student intern
- 2+ years in behavioral health/PRP/Health Home/rehab/community services
- ICANotes/CRISP familiarity; strong documentation; trauma-informed, culturally responsive practice; knowledge of COMAR/Medicaid
