Job Description
As a Case Manager, you will work with residents to develop and implement individualized Service Plans, provide referrals to resources, and advocate on behalf of residents. You will also facilitate therapeutic groups and community-building activities, and maintain up-to-date documentation in resident files.
Requirements
- High School Diploma + 4 years’ experience
- AA degree in similar field + 3 years’ experience
- CHW Certificate/CADC + 2 – 4 years’ experience
- BA/BS degree in similar field + 2 – 4 years’ experience
- MA/MS degree in similar field + 1 year experience
