Job Description
Perform intake assessments, outreach, and service planning for residents, and facilitate therapeutic and psycho-educational groups.
Requirements
- High School Diploma + 4 years’ experience
- AA degree in similar field + 3 years’ experience
- CHW Certificate/CADC + 2 – 4 years’ experience
- BA/BS degree in similar field + 2 – 4 years’ experience
- MA/MS degree in similar field + 1 year experience
- Knowledge of local social services and community resources
- Excellent charting, recordkeeping, and professional writing skills
- Computer skills and experience using Microsoft Outlook, Word, and other MS Office applications
- Strong interpersonal skills and ability to communicate effectively
