Job Description
Perform intake assessment, outreach to residents, develop and implement individualized Service Plans, assess eligibility for benefits and entitlements, advocate on behalf of residents, assist with conflict resolution, participate in wellbeing checks, develop and facilitate therapeutic groups, and maintain documentation.
Requirements
- High School Diploma + 4 years’ experience or AA degree in similar field + 3 years’ experience
- CHW Certificate/CADC + 2 – 4 years’ experience
- BA/BS degree in similar field + 2 – 4 years’ experience
- MA/MS degree in similar field + 1 year experience
