As a Community Reablement Worker, you will meet with individuals, families and carers to connect and listen. To deliver a community-based enablement and inclusion service, which support people to learn or relearn skills, enable individuals to become as independent as possible, reduce social isolation, and encourage people to be part of their community.
Requirements
- Relevant health/social care qualifications to NVQ Level 2 or equivalent and / or equivalent relevant experience
- Have relevant experience of working with adults in a social care setting and of working with other agencies within a fieldwork environment
- Understand the need to undertake need and risk assessments and the issues/needs that adults and their families/carers may have
- Have a good understanding of social care organisations and their roles and responsibilities as part of the wider community and voluntary sector
- Be a confident communicator who is skilled in negotiation, persuasion, assertiveness, and conflict management, and able to build effective working relationships
- Be able to recognise the limits of own accountability and responsibility, and to seek appropriate support and advice when required
- Experience of prioritising, working to timescales and working in a consistent way
- Be able to manage a caseload and work to clear objectives
- Knowledge of relevant legislation and policies, e.g. Personalisation, Self-directed Support
- Experience of administrative procedures including the use of computers
